Job Description

We are seeking a dedicated Remote Bilingual Care Coordinator to join our team and make a meaningful impact on patients' lives. In this role, you'll deliver top-notch patient care coordination services, working with a diverse panel of patients, especially those managing chronic conditions. You'll play an important part in connecting patients with the care they need, ensuring they receive consistent, personalized support.

Key Responsibilities

Conduct monthly care coordination calls with patients, healthcare providers, and pharmacies to ensure patients receive continuous care between office visits.
Create and update personalized care plans in collaboration with medical practices and healthcare providers.
Identify and address social determinants of health and care gaps, ensuring patients receive necessary referrals and services.
Empower patients to actively manage their health and well-being, providing clear guidance in their care plans.
Make 30-50 calls per day, reaching at least 15 patients to complete 20 minutes of care coordination services.
Ensure all interactions maintain patient privacy, confidentiality, and adhere to all relevant healthcare regulations.
Who We're Looking For
We are looking for a compassionate and skilled individual with the following qualifications:

At least 1 year of experience as a Certified or Registered Medical Assistant (CMA/RMA).
Fluent in both Spanish and English with excellent verbal and written communication skills.
Comfortable working with multiple EMR/EHR systems, Microsoft Office, and Google Suite.
Ability to think quickly and engage patients with professionalism and empathy.
Strong critical thinking skills to address patient needs, answer questions, and follow up appropriately.
Self-motivated and able to work independently in a virtual, fast-paced environment.
Experience in call centers, triage, or care coordination is a plus.
Familiarity with serving a geriatric population or case management is also an advantage.
Working Environment & Requirements

Ability to commit to a full-time schedule, Monday through Friday.
Must have a private, distraction-free home office setup to maintain patient confidentiality.
Reliable, high-speed internet connection (minimum 50 mbps upload speed).
A second monitor is required for efficient multitasking.
A company laptop and software will be provided for patient calls and related duties.
